Bankruptcy Paperwork
From Kathryn D. Peters, Attorney at Law
Please fill out the attached paperwork. Once it is completed, contact the office for an appointment to return it to the office. This appointment may take as much as an hour, depending on how complete your information is and how many creditors you have. You also need to bring the funds necessary to pay the retainer fee to this appointment in order for us to begin processing your petition. This takes us approximately 30-45 days.
It is your responsibility to accurately complete this questionnaire and answer all the questions we may have. We cannot even obtain the information necessary from your creditors. When you sign the bankruptcy paperwork, you are swearing under the penalties of perjury that the information you provide to us is correct. If it is false, it could result in your bankruptcy being dismissed or criminal charges being filed against you.
The attorney fees which we charge don’t anticipate information gathering. They are for preparing the paperwork the court requires, representing you in negotiations with your creditors and for representing you at the 341 creditors’ meeting. Any additional representation would result in additional fees being charged.
The payment necessary to cover the retainer fee is the initial payment on your account. If you decide not to file bankruptcy through this office after a file has been opened and the initial payment made, we will retain the payment you have made for services we have rendered. However, be kindly advised that an appointment with the attorney is required before an appointment can be set to return paperwork. Call us today to schedule an appointment at 812-479-0700.
